Prepare for the Initial Evaluation

Thank you for choosing REHAB AT WORK. We are pleased to have the opportunity to support your rehabilitation and your return to a healthier you!

In an effort to make your initial evaluation go smoothly, please;

  • Be on time for your appointment
  • Bring your health insurance card and a form of picture identification
  • Bring your completed registration paperwork
  • Bring your physician’s referral and prescription for therapy, as applicable.
  • Wear comfortable clothing and sneakers suitable for movement
  • Do not wear excessive jewelry

To view the forms listed below, you will need to download Adobe Reader. Please bring your completed forms with you to our office at the time of your visit. Fill out and submit the online forms. 

2. Digitally Sign Forms

With our new website, you now have the ability to fill out the forms digitally. You do not need any additional applications for this process. Simply click on each link below and fill out each form.
