What do I need for my first appointment?

  • Be on time for your appointment and arrange for child care, if needed
  • Bring the forms listed on the For Patients page, completed
  • Wear comfortable clothing for movement and bring a pair of shorts if you have a back, leg or foot injury
  • Wear comfortable and supportive shoes (no sandals, dress shoes, boots, etc.)
  • Bring all medications, records, and diagnostic test results
  • Continue to take all medications as prescribed, including those not related to your injury (i.e. high blood pressure medication)
  • Bring reading glasses, if needed
  • Leave excessive jewelry at home

What are the components of the Functional Capacity Evaluation FCE?

There are four main parts of the evaluation – the intake process, the clinical interview, the musculoskeletal exam and the functional and work simulation testing.

The three main components of the program are: musculoskeletal exercises, functional activities and worker performance.

Why do I need to bring photo ID?

In order to verify your identity, name, date of birth and current address.

Why did you need information about my job requirements if I was not hurt at work? 

We want to get you as functional as possible so we will need to know about your job, home and leisure activities.

Insurance Information

Do I have to pay a co-pay every time I come in?

Yes, you do if your insurance plan requires it. We will verify your insurance coverage and benefits prior to your first visit.

How do I make payments?

We accept payment in the form of cash, check, VISA or MasterCard.

Who owns your company and do you work for the insurance company?

REHAB AT WORK is an independent, therapist owned entity.

How long has REHAB AT WORK been in business?

REHAB AT WORK has been in business for over 30 years.

What kinds of problems and ages do you treat?

We mostly treat people with orthopedic injuries of all ages based on individual situations.

Are your therapists licensed?

Yes, all of our therapists are licensed in the state where they treat. In Maryland and Virginia, per regulations, our clinicians are certified with the Maryland Workers’ Compensation Commission.

What are your office appointment hours?

We schedule appointment times from 8:00 to 3:30 Monday to Friday.

How can I get a copy of my records? 

You have to submit a written request upon completion of your services. It will take three weeks for you to receive your report.
